Geriatric Care Coordinator

Timmins and District Hospital

Position Summary: Timmins and District Hospital is currently seeking a Geriatric Care Coordinator for the Emergency Department. Unit. Reporting to CCU/ER, the Acute Medical Director, and The Geriatric Care Coordinator will collaborate with members of the allied health team and external partners to ensure high-quality, effective, and efficient services and care is provided to the aging population. Their focus will be on those frail seniors who are at risk of suffering adverse events, loss of independence, and admission to hospital or long-term care. Geriatric Care Coordinator will provide expert and targeted assessment and treatment services in the ED; they enhance linkage with hospital and community services and are advocates of the seniors they serve.  As well, in their role as geriatrics educators and capacity builders within the ED, the hospital, and the community, the Geriatric Care Coordinator helps to build the system’s capacity to ensure that frail seniors at risk receive the right care in the right place, at the right time. 

Position Responsibilities:

  • Implement standardized protocols to screen and assess high-risk seniors in the Emergency Department.
  • Assist in strengthening linkages with community services such as primary care, specialized geriatric services, community support services, CCAC, and long-term care.
  • Provide consultation to health care professionals and elderly patients regarding geriatric health issues
  • Assess, plan, implement, and evaluate care of elderly persons with complex geriatric health problems
  • Collaborate with healthcare      professionals to develop and implement a comprehensive plan of care
  • Assess the coping strengths of the elderly person and his/her family/support and plan intervention and/or intervene to assist the elderly person to achieve optimal independent functioning
  • Participate in quality improvement initiatives related to Senior’s Friendly Hospital initiatives and ED care.
  • The Geriatric Care Coordinator provides a specialized clinical resource for assessing and treating the frail elderly. His/her focus is on those frail seniors who are at risk of suffering adverse events, loss of independence, and admission to hospital or long-term care facilities.
  • Provide assessments, consultation, and advice to help facilitate emergency department care including acute symptoms, underlying health conditions, function, emotional and cognitive status, home environment issues, and home supports
  • The Coordinator provides expert and targeted assessment and treatment services; enhances linkages with hospital and community services and advocates for frail seniors.
  • The Coordinator is a geriatrics educator and capacity builder within the Emergency Department, the hospital, and the community, to ensure that frail seniors at risk receive the right care, in the right place, at the right time.
  • To facilitate the timely discharge of elderly patients from the ED and to link them to community resources.
  • To promote a model of best practice in Elder Care nursing and an elderly-friendly philosophy of care within the ED.
  • Effective use of the Standards of Nursing Practice and the Nursing Process in assessing, planning, implementing, and evaluating the care of the patient.

 

Position Qualifications:

  • Baccalaureate level preparation required. A Master’s degree in nursing or Gerontology is considered an asset.
  • Registered nurse in good standing with the College of Nurses of Ontario
  • 3-5 years of emergency experience
  • Additional training or experience in Gerontology is required, and/or a suitable candidate commits to complete an education plan for specialized Geriatric training
  • Working knowledge of Ontario Health and community resources
  • Knowledge and skills in functional assessment and the assessment of physical and mental illness of the elderly
  •  Evidence of strong clinical knowledge and decision-making
  • Ability to change, innovate, and be flexible in an ever-changing environment
  • Demonstrates excellence in interpersonal communication, critical thinking, and health teaching.
  • Ability to provide support and evidence-based interprofessional patient/family-centered care
  •  Demonstrated excellence in oral and written communication
  •  Demonstrates excellent work performance and attendance record.
  •  Bilingualism is considered an asset.
  • Must have had at least two doses of the COVID-19 vaccine.
