Pharmacist Diabetes Education Clinic

Timmins and District Hospital

Full-Time • Timmins Region

Salary Range: $80,000 - $100,000

Position Summary:

 Reporting to the Director of Clinical Services the individual will be responsible for providing

Counselling, training, comprehensive medication assessment, medication therapy management, education and support to individuals of all ages including their families/supports affected by diabetes. Working in collaboration with the multidisciplinary team of Physicians, Registered Nurses, Registered Dieticians and Registered Social Worker in an outpatient setting providing individual consultation, group education classes and access to specialized diabetes services including diabetes and pregnancy counselling, a provincially designated pediatrics diabetes program and insulin pump program. 

Position Requirements: 

  • Current registration with the Ontario College of Pharmacists (OCP) and in good standing required.
  • Certified Diabetes Educator (CDE) or working towards certification is required.
  • Recent experience (within last 2 years) in Diabetes Education for adults and children required.
  • Experience in developing diabetes educational resources and programs for both adults and children.
  • Knowledge of adult and paediatric teaching and learning principles combined with recent teaching experience with diabetes population required.
  • An awareness and sensitivity to various cultural needs.
  •  Ability to function as a client advocate.
  • Must have the ability to organize work according to priorities and t adjust this prioritization as required.
  • Must have excellent assessment, communication (verbal and written) and observational skills.
  • Must be able to perform physical demands of the position.
  • Must have the ability to work independently and collaboratively with all member of the multidisciplinary team, clients, families, regional support services and community partners.
  • Must have the ability to provide health teaching to individuals and groups.
  • Must have the ability to assume responsibility for patient care in a total patient care climate.
  • Must have a good work ethic and work record demonstrating regular attendance at work.
  • Must possess the ability to provide effective services to clients/patients in both English and French. Individuals who do not meet the language requirements of this posting are invited to submit their resume for future consideration.
