We are looking for a compassionate and skilled Social Worker to join our primary care team, providing essential care and system navigation to patients who do not have access to care and those who are experiencing or are at risk of experiencing homelessness This position will work alongside an interdisciplinary team to address the social determinants of health. This role is critical in ensuring equitable access to healthcare for underserved populations.
Primary Responsibilities
§ Conduct interviews and evaluations to assess patient needs and identify barriers to healthcare, including social determinants of health.
§ Develop and manage individualized care plans addressing both healthcare and social support needs, ensuring patients receive appropriate services.
§ Educate and advocate for patients to ensure they receive timely and equitable healthcare services. Empower them to navigate the healthcare system and access essential resources.
§ Work closely with healthcare providers and community organizations to ensure coordinated care. Refer patients to relevant services.
§ Help patients access services (e.g. transportation, language support, and financial assistance), addressing barriers to care.
§ Engage in professional development, stay updated on policies, and contribute to team efficiency through participation in meetings, projects, and education sessions.
§ Protects patient confidentiality in accordance with legislation, TAFHT policies and standards established by their College.
§ Sound theoretical knowledge.
§ Ensure adherence to PHIPA.
Education, Qualification and Skills
§ Bachelor’s or Master’s degree in Social Work from a recognized institution.
§ Registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW), or any other recognized institution.
§ Minimum of 2 years of experience, preferably in a healthcare or community setting, with knowledge of community resources and social services.
§ Knowledge of Microsoft Office (Word, Excel, Outlook) and experience with electronic medical records (EMR).
§ Proficient in assessment, psychotherapy, and psychosocial counseling.
§ Experience working with marginalized populations and an understanding of the challenges related to accessing healthcare services.
§ Excellent communication and interpersonal skills, with a strong commitment to patient advocacy.
§ Ability to work independently and collaboratively as part of an interdisciplinary team.
§ Ability to prioritize and adapt in a fast-paced and constantly changing work environment.
§ Ability to speak and write in both official languages, English and French is an asset.
Additional Job Requirements
§ Must adhere to all TAFHT Policies and Procedures.
§ Must be legally entitled to work in Canada.
§ This position is in Timmins, ON, the position requires working in office and on site.
§ As a condition of employment, you are required to submit proof of COVID-19 vaccination.
§ Current Ontario Driver’s License.

This position reports directly to the Clinical Director. The location of this position could be the Administration, Algonquin West, 101 Mall, Algonquin East, Third Avenue site or any other location within Timmins. The employer reserves the right to modify the location/site. This position is a bargaining unit position, represented by OPSEU/SEFPO.
